Graduate Application Deadline Change Form

This form allows graduate coordinators to submit requests for changes to upcoming graduate program application term deadlines.

A single form can be submitted for multiple programs with the same deadline. Multiple forms will be needed for programs with differing deadlines.

If the program deadline you would like to update has already passed, please contact Emma Kopko at ekopko@ilstu.edu prior to completing this form.

By default, the Graduate School automatically denies submitted applications from students with missing required materials two weeks after the program deadline has passed. If you would like incomplete program applications to be denied prior to or following the standard two-week timeframe, please indicate the last day would you like the Graduate School to accept materials from applicants who are missing required documents in the form below.

Application deadline change requests should be submitted no less than two business days prior to the program's current deadline to allow time for the Graduate School to fulfill your request.
