Complete and submit this form at least two weeks prior to your last day of summer enrollment. If you have not already done so, complete and submit the 2025-2026 Free Application for Federal Student Aid (FAFSA).

We will always communicate with you electronically. Check your To-Do list in My.IllinoisState.edu to see if you have any outstanding documents that may prevent loans to be offered and/or disburse.

Federal Direct Loans must be accepted before your last summer class ends. After the last date of your scheduled enrollment has passed, any unaccepted loan will be removed.
